My suggestion of a well planned lesson is, for any level, to divide it into two parts: grammar first and then conversation (covering all the abilities mentioned above). Learning is easy, but unlearning things that have become bad habits is extremely difficult. This makes it all the more important to learn the right reflexes as early as possible.
